Attached is a blank MHSAA Form to take to the sports physical appointment. Current physical must be dated after 4/15/25 and submitted through the athlete registration site by the official fall sports start dates. 9th-12th Grade: Monday August 11th. / 7th-8th Grade Football: Monday August 18th / MS Cross Country & 7th-8th Girls Volleyball: Monday August 21st.
